Onychomycosis is a fungal infection of the nails caused by fungi called dermatophytes, and is considered hard to treat. A compound solution of itraconazole, ibuprofen, and DSMO is a new, safe, and effective treatment of onychomycosis that can be compounded by Davis Islands Pharmacy.



Itraconazole is widely used for the treatment of nail fungus, for its high rate of effectiveness: it’s quick penetration of the nail and nail bed, continued use in the nail for months after discontinuation of therapy, and it’s generally good safety profile. One of the newer triazole medication with a broad antifungal spectrum of treatment which includes dermatophytes, many nondermatophyte molds and various Candida species, this compound is usually taken once a day for 6 – 12 weeks.



The effectiveness of itraconazole in treating nail fungus increases when compounded together with other ingredients. Ibuprofen is a non-steroidal anti-inflammatory drug (NSAID) compounded with itraconazole, treating mild to moderate pain and inflammation caused by the underlying fungus. Ibuprofen is not without antifungal properties of its own, making it an ideal NSAID to compound with Itraconazole. Ibuprofen is also known to synergize the effect of other antifungal drugs on various fungal infections. When compounded with other chemicals, Ibuprofen disrupts the biofilm of different fungal infections, making the overall compound more effective compound.



Ibuprofen compounded with other ingredients into a topical solution has benefits when compared to taking the same drugs by mouth: you get the advantage of local, enhanced delivery to painful sites with a reduced incidence of gastrointestinal upset or bleeding when compared to the same drugs taken orally. Ibuprofen blocks the enzyme that produces prostaglandin (a hormone-like substance that participates in a myriad of body functions), which leading to lower levels of prostaglandins. Lower levels of prostaglandins in the body, in turn, reduce pain, decrease inflammation, and fever.



Adding one more ingredient, DMSO, to the formulation enhances the other antifungal medications to penetrate the nail and push the antifungal ingredients closer to the spot of the underlying infections.



Itraconazole – Ibuprofen – DMSO Nail Solution: Directions for Use



Generally, this compound is prescribed to be used once or twice daily. Apply to clean, dry affected nails using applicator brush. This solution is to be applied as directed. Usual directions are twice daily, once in the morning and once at bedtime. It’s easiest to take this compound after showering, to ensure you’re clean. This medication works best when used continuously until the fungal infection is eliminated. Most nail infections are cleared after one to three months of therapy. The nail must be given time to grow out and replace the damaged nail.



Itraconazole – Ibuprofen – DMSO Nail Solution Side Effects:



Generally, this compound is considered safe. It does not achieve significant levels in the blood, therefore it does not usually cause the types of side effects seen when the medication is taken by mouth. However if a rash or burning sensation occurs, discontinue use and contact your physician or pharmacist.



Do not use if allergic to itraconazole or ibuprofen. Additionally, alert your doctor is you are taking cilostazol and eletriptan, as the addition of this compound may result in elevated plasma concentrations.



Although rare, additional side effects can include hepatotoxicity, dermatologic rash, congestive heart failure, hypertension, nausea, vomiting, dizziness, and headache. Itraconazole is in pregnancy category C; therefore, it should not used by pregnant women or women contemplating pregnancy.
